Cefixime trihydrate
Indications/Uses
Upper resp tract (eg, bacterial pharyngitis, tonsilitis, otitis media, sinusitis); lower resp tract (eg, acute bronchitis); UTI (eg, acute uncomplicated cystitis & urethritis); uncomplicated gonorrhea.
Dosage/Direction for Use
Adult 400 mg once daily or 200 mg every 12 hr. Uncomplicated cervical/urethral gonococcal infections 400 mg as a single dose. Childn 8 mg/kg/day or 4 mg/kg every 12 hr. Typhoid 15-20 mg/kg/day as a single dose or divided into 2 equal doses every 12 hr for 7-14 days. Infections due to S. pyogenes At least 10 days.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity to cephalosporins. Renal impairment w/ CrCl <60 mL/min.
Special Precautions
History of hypersensitivity to penicillins. Predispose the patient to encephalopathy risk. Severe cutaneous adverse reactions eg, toxic epidermal necrolysis, Stevens-Johnson syndrome & drug rash w/ eosinophilia & systemic symptoms (DRESS). Drug-induced hemolytic anaemia. May cause acute renal failure including tubulointerstitial nephritis. Prolonged use may result in the overgrowth of non-susceptible organisms. Antibiotic-associated pseudomembranous colitis. Renal impairment. Not to be used during pregnancy or lactation. Premature or newborn infant.
Adverse Reactions
GI disturbances including diarrhea, stool changes; nausea, abdominal pain, dyspepsia, vomiting & flatulence; pseudomembranous colitis. Headache & dizziness. Hypersensitivity reactions in form of rash, pruritus, urticaria, drug fever & arthralgia. Thrombocytopenia, leukopenia & eosinophilia; changes in liver function test. Genital pruritus & vaginitis.
Drug Interactions
Enhance effects of anticoagulants, prolonged prothrombin time w/ or w/o bleeding may occur. False +ve reaction for glucose in urine w/ Benedict's or Fehling's solution or w/ copper sulphate test tab. False +ve in direct Coombs test.
