Digoxin
Indications/Uses
Supraventricular arrhythmias, particularly atrial fibrillation & in heart failure.
Dosage/Direction for Use
Loading dose: 750-1,500 mcg as single dose or in divided doses at 6-hrly intervals during initial 24-hr period if rapid digitalization is required. Maintenance: 125-250 mcg daily or may range from 62.5-500 mcg daily. Patient eg, w/ mild heart failure Loading dose may not be necessary & digitalization dose w/ 250 mcg once or bid.
Administration
May be taken with or without food.
Contraindications
Hypertrophic obstructive cardiomyopathy unless there is severe cardiac failure. Wolff-Parkinson-White syndrome or other evidence of accessory pathway especially if accompanied by atrial fibrillation.
Special Precautions
Partial heart block; sinus node disorders; acute myocarditis, acute MI, severe pulmonary disease. Renal impairment.
Adverse Reactions
Nausea, vomiting, anorexia diarrhea, abdominal pain; visual disturbances including blurred vision.
Drug Interactions
Increased risk of toxicity w/ drugs that cause electrolyte disturbances. Thiazides & loop diuretics may lead to cardiac arrhythmia. May cause hypokalemia w/ corticosteroids, β2 agonists (eg, salbutamol), amphotericin B, Na polystyrene sulfonate, carbenoxolone, dialysis. Hypercalcemia may increase toxicity & IV use of Ca salts. Serum conc may be increased by quinidine, amiodarone & propafenone. Other antiarrhythmics may have additive effects on adverse effects. β-blockers may potentiate bradycardia. Increased digoxin conc w/ Ca channel blockers.
