Indications/Uses
Antidote to folic acid antagonists. Palliative treatment of advanced colorectal cancer in combination w/ 5-fluorouracil.
Dosage/Direction for Use
Antidote for folic acid antagonists 6-12 mg IM every 6 hrly for 4 doses. Rescue after high-dose methotrexate therapy 10 mg/m2 orally or parenterally followed by 10 mg/m2 orally every 6 hr until serum methotrexate conc is <10-8 M. If at 24-hr following methotrexate administration the serum creatinine is increased by 50% or greater than the pre-methotrexate serum creatinine Immediately increase to 100 mg/m2 every 3 hr until serum methotrexate level is <5 x 10-8 M. Advanced colorectal cancer 200 mg/m2 by slow IV inj over a minimum of 3 min followed by 370 mg/m2 of 5-fluorouracil or 20 mg/m2 IV inj followed by 425 mg/m2 of 5-fluorouracil. Treatment is repeated daily for 5 days. 5-day treatment course may be repeated at 4 wk intervals for 2 courses & then repeated at 4-5 wk intervals.
Contraindications
Neonates & infants. Vit B12 deficiency anemia.
Special Precautions
Monitor serum methotrexate. Enhanced toxicity w/ 5-fluouracil. Debilitated colorectal cancer patients. Perform electrolytes & liver function tests prior to each treatment for the 1st 3 cycles, then prior to every other cycle. Discontinue treatment if there is clear evidence of tumour progression. Pregnancy & lactation. Childn.
Adverse Reactions
Allergic sensitization; anaphylactoid reactions, urticaria.
Drug Interactions
Phenobarb, phenytoin & primidone; methotrexate.
