Concise Prescribing Info
Complicated skin & skin structure infections, including diabetic foot infections, w/o concomitant osteomyelitis caused by Staph aureus (methicillin-susceptible & -resistant strains), Strep pyogenes or Strep agalactiae; uncomplicated skin & skin structure infections caused by Staph aureus (methicillin-susceptible strains only) or Strep pyogenes; community-acquired pneumonia caused by Strep pneumoniae (including multidrug resistant strains), including cases w/ concurrent bacteremia or Staph aureus (methicillin-susceptible strains only).
Dosage/Direction for Use
Complicated skin & skin structure infections, community-acquired pneumonia including concurrent bacteremia, nosocomial pneumonia Adult & adolescent ≥12 yr 600 mg every 12 hr for 10-14 days. Ped (birth to 11 yr) 10 mg/kg every 8 hr for 10-14 days. Vancomycin-resistant E. faecium infections including concurrent bacteremia Adult & adolescent ≥12 yr 600 mg every 12 hr for 14-28 days. Ped (birth to 11 yr) 10 mg/kg every 8 hr for 14-28 days. Uncomplicated skin & skin structure infections Adult 400 mg every 12 hr for 10-14 days. Adolescent 600 mg every 12 hr for 10-14 days. Ped 5-11 yr 10 mg/kg oral every 12 hr for 10-14 days; <5 yr 10 mg/kg every 8 hr for 10-14 days.
May be taken with or without food.
Hypersensitivity. Not for patients taking MAO inhibitors A or B (eg, phenelzine, isocarboxazid) or w/in 2 wk of taking such drugs. Patients w/ uncontrolled HTN, pheochromocytoma, thyrotoxicosis &/or patients taking directly & indirectly acting sympathomimetic agents (eg, pseudoephedrine), vasopressive agents (eg, epinephrine, norepinephrine), dopaminergic agents (eg, dopamine, dobutamine). Patients w/ carcinoid syndrome &/or taking SSRIs, TCAs, serotonin 5-HT1 receptor agonists (triptans), meperidine or buspirone.
Special Precautions
Myelosuppression (including anemia, leukopenia, pancytopenia & thrombocytopenia). Monitor wkly complete blood count, particularly in patients receiving linezolid for >2 wk, those w/ preexisting myelosuppression, receiving concomitant drugs that produce bone marrow suppression, or w/ chronic infection who have received previous or concomitant antibiotic therapy. Discontinue in case patient develop or have worsening myelosuppression. Not to be used for patients w/ catheter-related bloodstream or catheter-site infections. Not indicated for gram -ve infections. C. difficile-associated diarrhea. Hypoglycemia; caution use in diabetic patients for potential hypoglycemic reactions.
Adverse Reactions
Diarrhoea, nausea & vomiting, metallic taste, headache, insomnia, constipation, rashes, dizziness, fever, oral & vag candidiasis, & abnormal liver function tests. Lactic acidosis; convulsions.
Drug Interactions
Enhanced pressor activity w/ phenylpropanolamine, pseudoepherine, dopamine & adrenaline. Serotonin syndrome w/ SSRIs & dextromethorphan.
