Telmisartan
Indications/Uses
Essential HTN in adults. Reduction of CV morbidity in adults w/ manifest atherothrombotic CV disease (history of CHD, stroke or peripheral arterial disease); type 2 DM w/ documented target organ damage.
Dosage/Direction for Use
Essential HTN 40 mg once daily for 4-8 wk. Max: 80 mg once daily. CV prevention 80 mg once daily. Patients w/ renal impairment Initially 20 mg. Patients w/ mild to moderate hepatic impairment Should not exceed 40 mg once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Biliary obstructive disorders. Patients w/ DM or renal impairment (GFR <60 mL/min/1.73 m2). Severe hepatic impairment. Pregnancy (2nd & 3rd trimester).
Special Precautions
Not to be given to patients w/ cholestasis, biliary obstructive disorders or severe hepatic impairment. Risk of severe hypotension & renal insufficiency. Monitor K & creatinine serum levels. Intravascular hypovolaemia. Increase risk of hypotension. Patients w/ severe CHF or underlying renal disease including renal artery stenosis. Primary aldosteronism. Aortic & mitral valve stenosis, obstructive hypertrophic cardiomyopathy. Diabetic patients treated w/ insulin or antidiabetics. Hyperkalemia. Galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption. Black hypertensive population. MI or stroke. May affect ability to drive or use machines. Pregnancy & lactation.
Drug Interactions
Aliskiren. Digoxin. May increase serum K w/ K-sparing diuretics eg, spironolactone, eplerenone, triamterene, amiloride, K supplements or K-containing salt substitutes. Increase lithium conc. Reduce antihypertensive effects w/ NSAIDs (ASA, COX-2 inhibitors & non-selective NSAIDs). Risk of hypotension w/ thiazide or loop diuretics. Increased BP lowering w/ concomitant use of other antihypertensive. Reduction of antihypertensive effect w/ corticosteroid (systemic route).
