Tamoxifen citrate
Dosage/Direction for Use
Adult including elderly 20-40 mg daily in divided doses bid or as single dose once daily.
Administration
May be taken with or without food.
Contraindications
Special Precautions
Increased incidence of endometrial cancer & uterine sarcoma. May affect the development of endometrial pathology including neoplasia. Suppressed menstruation. 2nd primary tumours. May increase risk of microvascular flap complications. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Concomitant use w/ potent CYP2D6 eg, paroxetine, fluoxetine, quinidine, cinacalcet or bupropion. Premenopausal women. Fatigue may occur & should be observed when driving or using machinery. Not recommended during lactation. Not recommended in childn (<5 yr in early stages).
Adverse Reactions
Fluid retention; hot flushes; nausea; skin rash; vag bleeding & discharge; fatigue. Uterine fibroids; anaemia; hypersensitivity reactions; ischaemic cerebrovascular events; headache; light headedness, sensory disturbances (including paraesthesia & dysgeusia); cataracts; retinopathy; vomiting, diarrhea, constipation; changes in liver enzymes; fatty liver; alopecia; leg cramps, myalgia; pruritus valvae; endometrial changes (including hyperplasia & polyps); elevated triglycerides; thromboembolic events (including DVT, microvascular thrombosis & pulmonary embolism).
Drug Interactions
Increase in anticoagulant effect w/ coumarin-type anticoagulants. Increased risk of thromboembolic events w/ cytotoxic agents. Reduced plasma levels w/ CYP3A4 inducing agent (rifampicin). Reduced effect w/ SSRI antidepressants (eg, paroxetine) or w/ potent CYP2D6 inhibitors (eg, paroxetine, fluoxetine, quinidine, cinacalcet or bupropion).
