Cefepime HCl
Indications/Uses
Infections due to susceptible organisms eg, UTI, resp tract & skin infections.
Dosage/Direction for Use
Administer IV over approx 30 min. Adult Moderate to severe pneumonia 1-2 g IV 12 hrly for 10 days. Empiric therapy for febrile neutropenic patient 2 g IV 8 hrly for 7 days or until resolution of neutropenia. Mild to moderate uncomplicated or complicated UTI including pyelonephritis 0.5-1 g IV/IM 12 hrly for 7-10 days. Severe uncomplicated or complicated UTI including pyelonephritis &/or moderate to severe uncomplicated skin & skin structure infections 2 g IV 12 hrly for 10 days. Complicated intra-abdominal infections (used in combination w/ metronidazole) 2 g IV 12 hrly for 7-10 days. Normal renal function CrCl >60 mL/min 500 mg 12 hrly, 1 g 12 hrly, 2 g 12 hrly, 2 g 8 hrly. Renal insufficiency CrCl 30-60 mL/min 500 mg 24 hrly, 1 g 24 hrly, 2 g 24 hrly, 2 g 12 hrly; 11-29 mL/min 500 mg 24 hrly, 500 mg 24 hrly, 1 g 24 hrly, 2 g 24 hrly. <11 mL/min 250 mg 24 hrly, 250 mg 24 hrly, 500 mg 24 hrly, 1 g 24 hrly. Continuous ambulatory peritoneal dialysis 500 mg 48 hrly, 1 g 48 hrly, 2 g 48 hrly, 2 g 48 hrly. Hemodialysis 1 g on day 1, then 500 mg 24 hrly, 1 g 24 hrly. Febrile neutropenia 1 g on day 1 then 1 g 24 hrly.
Contraindications
Hypersensitivity to cefepime or cephalosporins, penicillins or other β-lactam antibiotics.
Special Precautions
Prolonged use may result in overgrowth of nonsusceptible microorganisms. Monitor prothrombin time. History of GI disease particularly colitis. Renal or hepatic impairment; +ve Coombs' test. Pregnancy & lactation. Childn <2 mth. Elderly.
Adverse Reactions
Encephalopathy including confusion, hallucinations, stupor & coma; myoclonus & seizures; pseudomembranous colitis.
Drug Interactions
Increased potential of nephrotoxicity & ototoxicity w/ high doses of aminoglycosides. Nephrotoxicity may occur w/ potent diuretics eg, furosemide.
