Dopamine HCl
Indications/Uses
MI, trauma, endotoxic septicemia, shock due to surgery, renal failure & chronic cardiac compensation as in refractory congestive failure, oliguria, anuria, hypotension & other circulatory disorder due to inadequate cardiac output.
Dosage/Direction for Use
Adult Begin infusion at doses of 2-5 mcg/kg/min. More severe cases Initiate at 5 mcg/kg/min & increased gradually in 5-10 mcg/kg/min increments up to 20-50 mcg/kg/min.
Contraindications
Pheochromocytoma & uncorrected tachyarrhythmias or ventricular fibrillation.
Special Precautions
Pressor therapy is not a substitute for replacement of blood, plasma, fluids &/or electrolytes. Correct blood vol depletion before dopamine therapy is instituted. Monitor patients w/ history of occlusive vascular disease (eg, atherosclerosis, arterial embolism, Raynaud's disease, cold injury, diabetic endarteritis, or buerger's disease). Ischemic heart disease. Pregnancy & lactation. Childn.
Adverse Reactions
Ectopic heartbeats, tachycardia, angina, palpitation, vasoconstriction, hypotension dyspnea, nausea, vomiting & headache.
Drug Interactions
Avoid cyclopropane & halogenated hydrocarbon. Potentiated effects & duration of action w/ MAOIs. Antagonised renal vasodilation w/ adrenergic blocking agents (eg, phenothiazines & butyrophenones). May increase sympathomimetic action w/ guanethidine, TCA, reserpine, sympathomimetics, thyroid hormone, antihistamines. α or β-adrenergic blocking agents may induce BP change.
