Vortioxetine HBr
Indications/Uses
Major depressive disorder in adults.
Dosage/Direction for Use
Initially 10 mg once daily. May be increased to a max of 20 mg once daily or decreased to a min of 5 mg once daily. Treatment for at least 6 mth after depressive symptoms resolve.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Concomitant use w/ non-selective MAOIs or selective MAO-A inhibitors.
Special Precautions
Closely monitor patients w/ history of suicide-related events or those exhibiting a significant degree of suicidal ideation prior to commencement of treatment during therapy. History of seizures or unstable epilepsy. Discontinue treatment if patient develops seizures or for whom there is an increase in seizure frequency. Monitor patient for the emergence of signs & symptoms of serotonin syndrome or neuroleptic malignant syndrome. History of mania/hypomania. Concomitant use w/ anticoagulants &/or drugs known to affect platelet function & patients w/ known bleeding tendencies/disorders. Patients w/ liver cirrhosis or concomitantly treated w/ drugs known to cause hyponatremia. Severe renal & hepatic impairment. Patients should exercise caution when driving or operating hazardous machinery. Pregnancy & lactation. Childn <18 yr. Elderly (especially at risk of hyponatremia).
Adverse Reactions
Nausea. Abnormal dreams; dizziness; diarrhoea; constipation, vomiting; generalised pruritus.
Drug Interactions
Risk of serotonin syndrome w/ irreversible non-selective MAOIs; moclobemide; linezolid; selegiline, rasagiline; tramadol, sumatriptan & other triptans; antidepressants w/ serotonergic effect & herbal remedies containing St. John's wort (Hypericum perforatum). Decreased seizure threshold w/ antidepressants (eg, TCAs, SSRIs, SNRIs) neuroleptics (phenothiazines, thioxanthenes & butyrophenones), mefloquin, bupropion, tramadol. CYP450 inhibitors & inducers. Decreased AUC of vortioxetine w/ rifampicin; carbamazepine, phenytoin. Potential increased risk of bleeding w/ anticoagulants or antiplatelet drugs. Enhanced effect w/ lithium or tryptophan.
