Indications/Uses
Various neoplastic disorders including acute lymphocytic leukemia, acute myelogenous leukemia, acute non-lymphocytic leukemia, chronic myeloid leukemia, acute promyelocytic leukemia, meningeal neoplasms. Treatment of non-Hodgkin's lymphoma, herpes virus infection & psoriasis.
Dosage/Direction for Use
Treatment of leukaemia, lymphoma, management of myelodysplasia Induction of remission: Adult & childn 100-200 mg/m2 or 2-6 mg/kg body wt by IV infusion over 24 hr or by IV inj for 5-7 days or until bone marrow hypoplasia occurs. Maintenance: 1-1.5 mg/kg IV/IM/SC once-twice wkly. Treatment of refractory disease Up to 3 g/m2 by continuous IV infusion every 12 hr for 6 days. Leukaemic meningitis 10-30 mg/m2 intrathecally every 2-4 days. To induce differentiation of malignant cells in patients w/ myelodysplastic syndromes 10 mg/m2 bid SC.
Contraindications
Severe bone marrow depression. Via IM inj. 1st trimester of pregnancy, lactation.
Special Precautions
Tumor cell infiltration of the bone marrow. Patients w/ underlying CNS disease.
Adverse Reactions
Nausea, vomiting, GI mucositis, syndrome characterised by fever, myalgia, bone pain & occasionally chest pain, maculopapular rash & conjunctivitis in childn. Myelosuppression w/ megaloblastic changes in erythropoiesis of the bone marrow & reticulocytopenia in the peripheral blood. Noncardiogenic pulmonary edema, cerebellar dysfunction, rash, hepatotoxicity.
Drug Interactions
Aspirin, alcohol, digoxin, dipyridamole, methotrexate. Live virus vaccines.
