Indications/Uses

Monotherapy or adjunctive therapy of partial seizures w/ or w/o secondary generalisation; neuropathic pain.

Dosage/Direction for Use

Adult & childn >12 yr Epilepsy 900-1,800 mg/day tid. Dosage titration: Titrate 300 mg or 400 mg up to 1,800 mg/day in 3 divided doses. Neuropathic pain Initially 300 mg dose on day 1, 600 mg/day on day 2 bid & 900 mg/day on day 3 tid. Dosage titration: Titrated up to 1,800 mg divided tid. Maintenance dosage: 1,800-3,600 mg/day. Renal impairment CrCl ≥60 mL/min 900-3,600 mg/day, >30-59 mL/min 400-1,400 mg/day, >15-29 mL/min 200-700 mg/day, 15 mL/min 100-300 mg/day <15 mL/min ½daily dose of CrCl 15 mL/min patients dose.

Overdosage

Special Precautions

Renal impairment, patients undergoing hemodialysis, DM. Avoid abrupt w/drawal. False positive results w/ urinary protein test. May increase risk of suicidal behavior/thoughts; monitor for signs/symptoms of depression, suicidal tendencies & other behavioral changes. Concomitant use w/ other sedative drugs or ethanol. Ped patients 3-12 yr. May impair ability to drive or operate machinery. Pregnancy & lactation. Elderly.

Adverse Reactions

Somnolence, dizziness, ataxia, fatigue. Peripheral edema, vasodilation; headache, abnormal thinking, amnesia, depression, dysarthia, nervousness, abnormal coordination, twitching, hyperesthesia; pruritus, rash; hyperglycemia; diarrhea, nausea/vomiting, abdominal pain, xerostomia, constipation, wt gain, dyspepsia, flatulence, dry throat, dental abnormalities, appetite stimulation; impotence; leukopenia, decreased WBC; tremor, weakness, abnormal gait, back pain, myalgia, fracture; nystagmus, diplopia, blurred vision, conjunctivitis; otitis media; rhinitis, pharyngitis, cough.

Drug Interactions

Reduce absorption w/ antacids. Reduce clearance w/ morphine. Increase AUC w/ hydrocodone. Increase absorption w/ naproxen. False +ve results of Ames N-Multistix SG dipstick test for urinary protein.
