Indications/Uses
Anaemia associated w/ chronic kidney disease in dialysis & non-dialysis patients.
Dosage/Direction for Use
Patients currently not treated w/ an erythropoiesis-stimulating agent (ESA) Patients not on dialysis: Initially 1.2 mcg/kg single SC inj once every mth. Alternatively, 0.6 mcg/kg single IV or SC inj once every 2 wk. Patients on dialysis: Initially 0.6 mcg/kg once every 2 wk as a single IV or SC inj. Patients currently treated w/ an ESA Previous wkly epoetin dose <8,000 U/wk Mircera 120 mcg/mth or 60 mcg once every 2 wk as single IV or SC inj, 8,000-16,000 U/wk Mircera 200 mcg/mth or 100 mcg once every 2 wk as single IV or SC inj, >16,000 U/wk Mircera 360 mcg/mth or 180 mcg once every 2 wk as single IV or SC inj. Previous wkly darbepoetin α dose <40 mcg/wk Mircera 120 mcg/mth or 60 mcg once every 2 wk as single IV or SC inj, 40-80 mcg/wk Mircera 200 mcg/mth or 100 mcg once every 2 wk as single IV or SC inj, >80 mcg/wk Mircera 360 mcg/mth or 180 mcg once every 2 wk as single IV or SC inj.
Contraindications
Hypersensitivity. Uncontrolled HTN.
Special Precautions
Evaluate Fe status prior to & during treatment to ensure effective erythropoiesis. Monitor BP & Hb levels. Bone marrow exam for diagnosis of pure red cell aplasia (PRCA) if sudden drop of Hb is associated w/ reticulocytopenia & anti-erythropoietin Ab (AEAB). Discontinue therapy if PRCA is diagnosed. Do not switch to another ESA if patient has PRCA. Adequately control BP prior to, at initiation of & during treatment. Haemoglobinopathies, seizure, platelets >500 x 109/L. May stimulate growth of any type of malignancy. Chronic blood loss, bone marrow fibrosis, severe Al overload due to renal failure treatment, folic acid or vit B12 deficiencies & haemolysis. Patient <18 yr. Pregnancy & lactation.
