Norepinephrine bitartrate
Indications/Uses
Shock which persists after adequate fluid vol replacement; severe hypotension.
Dosage/Direction for Use
IV infusion Dilute 4 mL (4 mg) in 1,000 mL 5% dextrose soln. Infusion rate: Initially 2-3 mL/min. Maintenance: 0.5-1 mL/min.
Contraindications
Hypotension from hypovolemia except as emergency measure to maintain coronary & cerebral perfusion until vol can be replaced. Mesenteric or peripheral vascular thrombosis unless it is a lifesaving procedure. During anesth w/ cyclopropane or halothane (risk of ventricular arrhythmias).
Special Precautions
HTN, asthma. Inj into a large vein particularly an antecubital vein. Avoid veins in legs of elderly. Pregnancy & lactation. Childn.
Adverse Reactions
Ischemic injury, bradycardia, arrhythmia, anxiety, transient headache, resp difficulty, extravasation, necrosis at inj site, plasma vol depletion (prolonged use).
Drug Interactions
β-blockers, cyclopropane & halothane anesth, COMT inhibitors, MAOIs or TCAs.
