Vancomycin HCl
Indications/Uses
Treatment of severe staphylococcal (including MRSA) infections eg, endocarditis, bone infection, septicemia, lower resp tract & skin infections. Treatment of severe staphylococcal infection in penicillin-allergic patient.
Dosage/Direction for Use
Severe infection caused by Staph spp including MRSA & other vancomycin-susceptible organism Adult 500 mg 6 hrly or 1 g 12 hrly over IV 60 min. Childn 10 mg/kg 6 hrly or 20 mg/kg 12 hrly over 60 min. Neonate/infant 15 mg/kg initially, followed by 10 mg/kg 12 hrly & 12 hrly for infant 1 wk-1 mth. Infection in neutropenic patients 20 mg/kg 8 hrly.
Contraindications
Hypersensitivity to vancomycin.
Special Precautions
Necrosis or severe pain in IM administration. Impaired renal or auditory function, elderly & infant especially in combination w/ aminoglycosides. Pregnancy & lactation.
Adverse Reactions
Neutropenia, renal failure, hearing loss, anaphylaxis. Redneck or red man syndrome, inflammation at inj site following rapid administration.
Drug Interactions
Anesth, concomitant use w/ aminoglycoside may increase nephrotoxicity, other neurotoxic/nephrotoxic drugs.
