Indications/Uses
Erectile dysfunction in adult males.
Dosage/Direction for Use
Recommended dose: 10 mg, may be increased to 20 mg if 10 mg does not produce adequate response. Taken at least 30 min prior to sexual activity. Patients who anticipate frequent use 5 mg once daily at approx the same time daily, may be decreased to 2.5 mg. Max dose frequency: Once daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Concomitant use w/ organic nitrates; guanylate cyclase stimulators eg, riociguat. Men w/ cardiac disease for whom sexual activity is inadvisable. Patients w/ MI w/in the last 90 days; unstable angina or angina occurring during sexual intercourse; NYHA Class 2 or greater heart failure in the last 6 mth; uncontrolled arrhythmias, hypotension (<90/50 mmHg), or uncontrolled HTN; stroke w/in the last 6 mth. Patients who have loss of vision in 1 eye caused by non-arteritic arterior ischemic optic neuropathy.
Special Precautions
Medical history & physical exam prior to treatment. Patients w/ pre-existing CV risk factors. Discontinue in case of sudden visual defect. Decreased or sudden hearing loss. Priapism & anatomical penile deformation. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. Severe renal & hepatic insufficiency. Concomitant use w/ potent CYP3A4 inhibitors (ritonavir, saquinavir, ketoconazole, itraconazole, erythromycin); other PDE5 inhibitors or other treatments for erectile dysfunction. Not indicated in women. No relevant use in paed population.
Adverse Reactions
Headache, flushing, nasal congestion, dyspepsia, back pain, myalgia, pain in extremity.
Drug Interactions
Increased plasma conc w/ CYP3A4 inhibitors (eg, ritonavir, saquinavir, ketoconazole, itraconazole, erythromycin & grapefruit juice). Reduced plasma conc w/ CYP3A4 inducers (eg, rifampicin, phenobarb, phenytoin & carbamazepine). Augmented hypotensive effects of nitrates, α-blocker (eg, doxazosin), riociguat. Use w/ caution when co-administered w/ 5-α reductase inhibitors. Increased bioavailability of ethinylestradiol.
