Desvenlafaxine succinate
Dosage/Direction for Use
Major depressive disorder 50 mg once daily. Patient w/ 24-hr CrCl <30 mL/min or end-stage renal disease (ESRD) 50 mg every other day. Moderate to severe vasomotor symptoms associated w/ menopause eg, hot flushes, night sweats 50 mg once daily for 7 days then titrate to 100 mg once daily. Patient w/ 24-hr CrCl <30 mL/min or ESRD 100 mg every other day.
May be taken with or without food: Swallow whole, do not dissolve/divide/chew/crush. Take at the same time each day.
Concomitant MAOIs or w/in at least 14 days of discontinuing treatment w/ MAOIs.
Special Precautions
Avoid abrupt cessation of treatment. Clinical worsening & suicidality. History of mania or hypomania. Patient w/ raised IOP, at risk of narrow-angle glaucoma or predisposed to bleeding. CV, cerebrovascular, lipid metabolism & seizure disorders. Underlying conditions that may be compromised by increases in BP. Regular monitoring of BP. May impair ability to drive or operate machinery. Childn <18 yr. Pregnancy, lactation.
Adverse Reactions
Nausea, dry mouth, constipation, fatigue, dizziness, insomnia, hyperhidrosis, palpitations, tachycardia, tinnitus, vertigo, blurred vision, mydriasis, diarrhea, vomiting, chills, asthenia, feeling jittery, irritability, wt changes; increased BP & blood cholesterol, decreased appetite, musculoskeletal stiffness, somnolence, tremor, paraesthesia, dysgeusia, attention disturbance, anxiety, abnormal dreams, nervousness, decreased libido, anorgasmia, abnormal orgasm, urinary hesitation, erectile dysfunction, delayed ejaculation, yawning, rash, hot flush, ischaemic cardiac events.
Drug Interactions
Serotonergic drugs, MAOIs, CNS-active drugs, lithium, sibutramine, tramadol, St. John's wort, linezolid, tryptophan supplements, alcohol, CYP3A4 inhibitors, drugs metabolized by CYP3A4.
